Emergencies
In case of emergency, call our number at 810-230-2280. After hours and on weekends, our phones are answered by an answering service, which relays a message to our on call physician automatically. If the situation is critical, call an ambulance or go immediately to the nearest Emergency Department at Hurley, McLaren or Genesys Regional Hospitals or the nearest facility to you if you are traveling.

John J. Bauer, M.D., uses the main medical centers located in the Flint and Grand Blanc area. Our team offers many of its urological services at these facilities. These services include emergency room consultations and procedures, inpatient consults, outpatient surgery, and inpatient surgery.

Insurance, Payment, Billing Information
John J. Bauer, M.D., is a provider for Medicare and most other major insurance plans. Patients should call the office to make sure their insurance plan is accepted by our providers. Our office physicians are not providers for Blue Cross Blue Shield-Traditional plans, however, we do provide care to patients with this plan. Fees are collected from the patient and then a bill of services is provided for the patient to send to BCBS for reimbursement.
We provide insurance billing for insurance plans we accept. Anything not covered by insurance will be your responsibility.
We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment; you are fully responsible for payment of the co-pay before any services are rendered. If there is a deductible on your insurance policy, you are responsible to make prompt payment if our office is not reimbursed for services rendered. When necessary, our staff will work closely with patients who require a payment plan. We accept major credit cards for payment (Visa/MasterCard). We will automatically bill your insurance carrier for you, and each month you will receive a statement outlining the activity on your account if it is active. Our fees are reasonable and are typically near the 50th percentile for the United States. Please address any questions concerning specific fees prior to any procedure or testing that you may undergo.
Questions concerning billing, accepted insurances or professional fees should be addressed to our Accounts Manager during office hours. Questions regarding your plan's coverage and benefits should be directed to your employer or insurance company.

Telephone Policy
Our physicians set aside time during the day to return non-emergent phone calls, however, this is usually at the end of the day. This policy insures that while we are seeing you at our office, there will be a minimum of interruptions. Please leave a daytime and evening phone number with the message.
Laboratory results are mailed to you shortly after your appointment. You will be called and a follow-up appointment will be made if there is an abnormal lab value. All tissue biopsies such as bladder, prostate, penile foreskin, and operative specimens will require at least five to ten (5-10) working days for results to return and are typically discussed at your next follow-up visit. If you do not receive a call regarding these results within two (2) weeks, you should call the office to inquire what their status is. Sometimes specimens are in need of second opinions or re-cuts for accurate diagnosis.
We request that you call us for routine medication prescription refills during business hours. After hours you may leave a message on the recorder, however, it will not be dealt with until the next day.

Our regular office hours are Monday through Friday from 8:30 a.m. - 4:30 p.m.
If the doctor is delayed because of an emergency or a delay at the hospital, we make every effort to call you prior to your appointment.

What to Bring to Your Appointment
- Your insurance card
- Physicians referral forms if required by insurance
- A list of current prescription,over-the-counter, or herbal medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- Any recent x-rays or appropriate records you may have
- Please bring the "new" patient or "follow-up" patient questionnaires that were sent to you in the mail, or that you can print out from links provided below for all patient forms. Show up to the appointment 15 minutes early so that this information can be transferred into your electronic medical record.

Patient Forms/ Questionnaire Mailings
New patients will need to fill out information about their health history and their registration/payment information. You can have these forms mailed to you, or you can print them from links below.
- Please bring these questionnaires to the office 15 minutes prior to your visit so that the data can be put into the electronic medical record system. Printable pages of these forms are available at the links below:
- Depending on your reason for referral you will be sent additional questionnaires or you may, instead, choose to print them from here by going to these links:
- All follow-up patients, except those returning for results or further diagnostic work-ups for a very recent primary visit, will receive an augmented questionnaire in the mail that merely seeks changes in the registration or history data from the original visit. Consultation Summary Reports
- You will receive a hard copy of your consultation report before you leave the office, an electronic copy will be e-mailed to your referring physician if he uses e-mail. A hard copy will be mailed to your physicians office within 24 hours for up-to-date record keeping.
- Additional hard copies will be made for $15.00 per copy at the request of the patient.
Disability Forms
- There is a $20.00 fee for each disability form that the doctor completes.
- If you are applying for disability through the Social Security system or your disability insurance company and require an assessment by our office, we have a $100.00 fee for this service.
